A Subject Access Request (SAR) is a request, under the Data Protection Act 2018, to an organisation asking for a copy of the personal data that it holds about you.
An SAR enables you to understand how and why an organisation is using your data and to check that they are doing it lawfully.
PLEASE NOTE: Once your request has been completed, the information provided will be available for a period of three months, after which time it will be deleted in line with our retention process. Downloading this information is your responsibility and you will receive email notification reminders regarding deletion. Thank you for your attention in this matter.

How to make a request
You can make a request for your health records here by creating an account and logging into the portal.

Your portal account will allow you to view the request status, any information relating to your request, and enable you to download your requested information.

WARNING: Please note due to high demand beyond the remit of our small administration teams we are currently facing a significant backlog of requests. We are unable to adhere to the one month time frame for all requests. This is unavoidable.

If you would like to receive your records electronically, please create an account or login with your existing credentials. This is the fastest and most efficient way of making a subject access request. With an account you will be able to answer any questions we have concerning your request and track the progress we are making.
Please make sure you have two forms of ID before submitting the request and that you select the application form which matches the health records you are asking to receive.
