MSE Access to Records

Summary

Welcome

A Subject Access Request (SAR) is a request, under the Data Protection Act 2018, to an organisation asking for a copy of the personal data that it holds about you.

An SAR enables you to understand how and why an organisation is using your data and to check that they are doing it lawfully.

How to make a request

You can make a request for your health records here by creating an account and logging into the portal.

Please find a user guide here.

Your portal account will allow you to view the request status, any information relating to your request, and enable you to download your requested information.

Please find further information about acceptable forms of ID here.

Please find our Frequently Asked Questions here.

Please only complete ONE application form for any records you require.

How did we do? Please let us know- we value your feedback:

Look out for the form once your request has been completed, or use the link on our page www.mse.nhs.uk/access-to-health-records
