A Subject Access Request (SAR) is a request, under the Data Protection Act 2018, to an organisation asking for a copy of the personal data that it holds about you.

An SAR enables you to understand how and why an organisation is using your data and to check that they are doing it lawfully. 


How to make a request

You can make a request for your health records here by creating an account and logging into the portal.

Your portal account will allow you to view the request status, any information relating to your request, and enable you to download your requested information.

If you would like to receive your records electronically, please create an account or login with your existing credentials. This is the fastest and most efficient way of making a subject access request. With an account you will be able to answer any questions we have concerning your request and track the progress we are making.

Please make sure you have two forms of ID before submitting the request and that you select the application form which matches the health records you are asking to receive.
